Teamwork, Communication, and Coordination Fuel the Community Paramedicine Program
Deaf Smith County Hospital District’s team-based approach is what makes the community paramedicine program successful, helping more patients recover at home after a hospital stay and avoid being readmitted.

Communication and coordination among the team of paramedics, EMTs, nurses, case managers, social workers, physicians, and Medicare wellness center staff are how the program has achieved a dramatic reduction in readmission rates from 33 percent to under 5 percent, the lowest in the region.
