Additional Diabetes Services
Managing diabetes may involve more than regular clinic visits. Your clinician at Hereford Health Clinic can help you understand which additional services may be appropriate and coordinate the next steps.
Regular lab work helps your clinician monitor your health and guide your care. HRMC offers testing close to home, with walk-ins welcome when you have a lab order.
For eligible Medicare patients living with diabetes and another long-term condition, chronic care management provides additional support with medications, appointments, and communication among members of your care team.
HRMC provides inpatient and outpatient care for diabetic foot ulcers and other slow-healing wounds. If you notice a sore or wound that isn’t healing, contact your clinician.
Some patients may benefit from additional support at home through HRMC’s community paramedicine program. Ask your health care professional whether the program may be appropriate for your needs.
