Deaf Smith County Hospital District Patient Survey Notice
Your care experience matters to us. That’s why we’ve partnered with J.L. Morgan & Associates to administer a confidential patient survey that will help us improve the care we provide.
If you recently had an emergency or clinic visit, inpatient stay, or had surgery in the hospital, you may be invited to participate in the patient survey. The surveys are completely confidential.
J.L. Morgan & Associates will contact selected patients in one of two ways:
- Email: Patients invited to participate in the survey will receive an email from patientexperience@patient-feedback.com inviting them to take the survey. The email will contain a link to the survey and an access code. Once you have entered the access code and your date of birth, you may complete the survey and submit your confidential responses by clicking ‘SAVE’. Please check your spam folder if you are unable to find the email in your inbox.
- Phone: If an email address was not provided, you may receive a phone call from J.L. Morgan inviting you to complete the survey over the phone. The phone number will be from a “205” area code in Alabama.
Participation is not required, but we value your feedback and appreciate you taking the time to complete the survey.
