Innovating Rural Health Care from the Ground Up: Hereford Regional Medical Center’s Community Paramedicine Program

Hereford EMS and Hereford Regional Medical Center are proud to share the success of our Community Paramedicine program, now featured in Panhandle Magazine.
Co-authored by Terry Hazlett, EMT-Paramedic, and Esmeralda Florez, LMSW, the article showcases how our EMS team is redefining rural health care — providing post-discharge support directly to patients in their homes, managing chronic conditions, and helping address food insecurity, mental health, and other key needs.
The program has helped reduce hospital readmissions from 55% to just over 3%, proving that innovative, community-based care works — especially in rural communities like ours.
